About the Role:

As a Community Care Worker, you will assist clients with general household tasks, day-to-day activities, and personal hygiene and care based on their individual needs. You will be part of a supportive team that positively impacts older individuals and improves their quality of life in the comfort of their homes.

Key Responsibilities:

• Assist with personal care tasks such as showering, grooming, dressing, and skin checks.
• Provide medication support by prompting, administering, and supervising medication.
• Assist with tasks within your scope of practice as directed by the Registered Nurse.
• Engage in social interactions and provide companionship.
• Transport clients to appointments and assist them during social outings.
• Perform domestic duties such as vacuuming, mopping, dusting, laundry, and meal preparation.
• Meet the goals, needs, and preferences outlined by the client and/or their advocate.
• Communicate, report, and document changes, deterioration, and concerns regarding clients to the Case Manager.
• Participate in staff training, appraisals, and team meetings.
• Contribute to quality improvement initiatives by gathering client feedback and adapting to workforce changes.
• Be actively involved in an incident management system.

Essential Criteria:

• A Certificate III in Individual Support or higher.
• A current unrestricted driver’s license with comprehensive insurance and access to your vehicle.
• Access to a reliable, registered, and insured motor vehicle.
• Access to a mobile device.
• Ability to work autonomously and adapt to the changing needs and schedules of our home care clients.

Desirable Skills:

• First Aid & CPR, Manual Handling Certificate.
• Understanding of Italian culture and language.
• Experience working with the elderly and knowledge of aged care issues.
